Descrição: Explores the practice of ethics consultation through an analysis of 9 ethics consultation cases at five clinical settings on the West Coast, drawing on participant observation; chart review; & interview data from doctors, nurses, & social workers (total N = 29). The availability & conditional nature of consultation & differing perspectives among participants & the nature & dynamics of consensus in consultation are emphasized. Analysis indicated that attending physicians largely control access to ethics consultation, & most of the problematic issues raised during ethics consultations are not "formally" ethical. Further, participants in the consultation process perceived & defined problematic issues differently. Consensus in ethics consultation reflects the asymmetrical relations among physicians, nurses, & patients as well as the difficulty of reconciling different values & treatment goals. It is concluded that, despite the aims of ethics consultation, local power structures & interpersonal factors affect the consultation process, & there is little input from sources external to the clinical environment.
