This Doctorate of Medical Science by compilation of published papers examines the utility of both single and multicentre clinical registries, demonstrating why they have become increasingly valued within interventional cardiology. The single centre studies show the benefit of recording detailed clinical and procedural data to evaluate systems of care and facilitate understanding of the pathophysiology of new, high-risk cardiovascular interventions such as carotid artery stenting and primary infarct angioplasty. The multicentre publications focus on the importance of well established, rigorous, methodology including consecutive patient enrolment using standardised data collection and statistical analysis, with both short and long term follow up. They also highlight how observational research adds to randomised controlled trials, demonstrating the clinical effectiveness and safety of cardiovascular intervention in the real world, particularly with respect to subgroups and new devices. In each paper, there is recognition of the limitations of registry-based research including selection bias, with discussion of the statistical methods to adjust for this. Finally, this thesis shows why interventional registries are hypothesis generating, and excellent for describing the natural history of cardiovascular disease and the long-term outcome after procedures.
